The following are the options you selected for the iCAN Summit – please check carefully then complete AND submit the Google Form below – please scroll within the form to complete all the questions.  Once that is done, click the blue Checkout button at the bottom of the page to make your payment on PayFast.  Your payment will be reconciled with your completed form and your place secured at the Summit.
